The Colton School District utilizes a number of different forms for record keeping. Those forms are listed below. If you have questions, or need assistance, please call 509-229-3386 or 509-229-3385.

There are a number of forms related to athletic participation. Some are general (and filled out by every student participating in athletics).
Other forms are specific to a particular sport.Before you may turn out for practice for any sport, the specific requirements and forms must be completed and turned in to the High School Office. Please turn in your forms on or before the sports meeting. All medical care plans and medications need to be into the high school office before the first practice to participate. If you need any forms please contact Tammy at school in August, 229-3386.

- Physical (note new physicals are required for freshmen)

ASB cards will also be available at the sports meeting.. Cards for Junior High and 6th grade are $20.00. Cards for High School are $25.00
